12. In the event of reorganization of a general education institution, the issuance of duplicate documents on education is carried out on the recommendation of the municipal education authority by the educational institution - the successor of the reorganized institution, in which the archive of the reorganized general education institution is stored; the document must be signed by the head, his deputy for educational work, three members of the pedagogical council of the general education institution and sealed with the seal of the general education institution.
In the event of liquidation of a general educational institution, the issuance of duplicates is carried out by the municipal education authority according to archival data and sealed with the seal of the relevant education authority.
13. A duplicate of the certificate of education is issued to the applicant without an attachment (insert) to it in the absence of a student's personal file, protocols of the examination committee and books for recording and recording documents on education after establishing the fact of graduation from a general education institution.
